  • Water well test (bacteria and nitrates/nitrites) $ 85

  • Re-inspection $ 35 per visit

  • Drainfield Replacement Site Evaluation $125 (Soil Boring done by Stephenson County Health Dept.)

  • Mobile $125 / Year
  • Temporary (in County) $ 55 - Non-Profit organizations pay no fee
  • Temporary (out of County) $ 75
  • Late Fee $ 50 - for Annual Permits - $25 for Temporary Permits
  • Pre-Opening / Plan Review $210
  • Change of Ownership $ 50

Body Art…

  • Operator’s Permit $ 70 for 2 years
  • Establishment $250 / Year

Drinking Water…

  • Water Sampling Bottles $ 12 (2 bottles)
  • Water Sampling Bottle Mailing $ 30
